How Long Does Whiplash Last? Recovery Timeline After a Car Accident

How Long Does Whiplash Last?

What recovery usually looks like after a car accident, why pain can show up a day or two later, and when to get checked.

The short answer

Most whiplash improves over the first few weeks to three months. It does not always fully go away: about half of people with whiplash still report some neck pain a year after the crash. Pain that is severe at the start, spreads into the arms, or comes with headaches or dizziness tends to last longer and should be examined early.

Why whiplash pain often shows up later

Many people feel fine at the scene and wake up stiff and sore the next morning. Adrenaline masks pain for hours, and injured muscles, ligaments and joints swell and tighten over the following day or two. Pain that starts the day after a crash is common and still worth having examined.

Feeling worse on day two does not by itself mean something serious is wrong. What matters is the pattern: pain that keeps building after the first few days, or new symptoms appearing, deserves an exam.

What the research says about recovery

A large international review of whiplash outcomes, the Bone and Joint Decade Task Force, found that about 50% of people with whiplash still report neck pain one year after their injury. People with more pain, more symptoms and more disability at the start recovered more slowly. Things about the crash itself, such as the direction of impact or the headrest type, predicted very little.

The same review found that fear of moving, passive coping and low mood are linked to slower recovery. Staying active, understanding what is injured, and getting clear guidance early all matter.

Most people can come straight to us. Go to the ER instead if you have any of these:

  • Numbness, tingling or weakness in your arms or legs
  • Severe pain directly over the bones in the middle of your neck
  • A severe or worsening headache, confusion, vomiting, or vision changes
  • Dizziness, trouble walking, or loss of balance
  • Any loss of consciousness, even briefly

Those can point to a fracture, a head injury or nerve compression, and they need emergency care first. Without them, a careful spine exam is the right next step, and we can usually see you the same day. We screen every crash patient for these warning signs at the first visit, and if we find one, we send you to the ER.

How we evaluate whiplash at McClean

We start with a history and a hands-on orthopedic and neurological exam. When the exam calls for it, we take X-rays in the office, including motion X-ray, which shows how the neck moves under load. Ligament injury that makes a joint loose can be invisible on the still X-rays most ERs take.

We measure your range of motion and record your findings at the first visit. We re-test you about every 4 weeks with the same measurements we took at the start. If you are not measurably better, we change the plan or refer you to the right specialist. We do not keep treating just to keep treating.

Your care is usually covered by your own auto insurance (PIP) in Utah, no matter who caused the crash. We verify your benefits before or at your first visit and bill your insurer directly. Read how Utah PIP works.

Common questions

Can whiplash show up days later?

Yes. Stiffness and pain often build over the first day or two after a crash, as adrenaline wears off and injured tissue tightens and swells. New symptoms that keep appearing after the first few days should be examined.

Does whiplash get worse before it gets better?

It often does over the first 24 to 72 hours. After that, most people slowly improve. Pain that keeps worsening after the first few days, or that spreads into your arms, needs an exam.

How long does mild whiplash last?

Mild whiplash often settles within a few weeks. Even so, research shows recovery is not guaranteed: about half of people with whiplash still report some neck pain a year later, so an early exam and a clear plan are worth it.

Can whiplash cause headaches?

Yes. Injury to the upper neck joints commonly refers pain into the head. Headaches after a crash should be examined, and a severe or worsening headache needs emergency care.

How long should I see a chiropractor after a car accident?

It depends on what is injured. Simple sprains often need 4 to 8 visits; injuries involving nerves or discs take longer. We re-test about every 4 weeks and change course or refer you if you are not measurably improving.
